Forms and guides for the 2010 4-H Reporting year which runs 9/1/09-8/31/10.
Fill-in Forms can be saved if the person filling it out has Adobe 8 (Reader or Professional) or higher, or saved by printing to Winpdf or to Microsoft Document Writer.

The old Blue Ribbon (Martech) Group Youth Enrollment Form (MS Word doc) matches the Ethnic & Race fields in 4-H Online, but the combination Races are extra. Only one "Other", "Mixed Race" or "more than one Race" entry for Hispanic and one for Not Hispanic are needed.
