What: Teen Healthy Lifestyles Summit

When: January 18-20, 2013

Where: 4-H Camp Ocala

Cost: $35 Full Time

Who knows better the needs of youth than youth? That is why we need you for our state-wide teen health summit. Florida 4-H in partnership with United Health Care will host a “Teen Healthy Lifestyles Summit” as a part of Leadership Adventure Weekend. This 3-day event will promote healthy decision making through activities and group discussions. Make a difference in your community and join us at the summit.

Objectives for this event:

To share the importance of healthy living and adoption of practices by youth
To increase the application of healthy behaviors among youth, their families, and their community
To actively engage young people in learning about healthy living topics

Because this is a grant based summit we are able to offer this event at a greatly reduced price.

Please see the list of enrolled participants below. At this time the event is full and we are no longer accepting registrations only wait list applicants. Please note, that your county is financially responsible for all participants listed from your county. If the participants listed below cannot attend, please inform Jordan Callaham (jordanc@ufl.edu) no later than December 21, to avoid financial penalty.

Registration Confirmation

Please make sure to confirm your registration by filling out the individual registration forms and mailing them to the Florida 4-H State Headquarters. All individual registration forms must be postmarked by December 21 to confirm the space. If form is not received, it will be considered a cancellation and the county will be billed the $35 fee.

Wait List

If your participants did not make the list, we will begin a wait list on a first come, first serve basis. Please email jordanc@ufl.edu to have participants added to this list.

Payment – $35/person

Personal checks will not be accepted. Please send one county check for all participants to Florida 4-H State Headquarters. Individual participants please contact your county agent for more information.

*Recommended, in our grant we said we try to mix youth with non-4-H youth for the summit
