Linda D. Cook Healthy Lifestyles Volunteer Scholarship

  • Covers up to five partial registrations for the Southern Region Leader Forum for volunteers with a 4-H Food and Nutrition Focus.
    Applications are due in the State 4-H Office by June 1.

Registration Deadline and Payment

  • All registration must be submitted to your county office by their set deadline. Georgia 4-H Medical Information & Release Form and full payment is due with the registration form by August 15, 2008, in the State 4-H Events Office at 3103 McCarty Hall B, Gainesville, FL, 32611. Checks should be made payable to: Florida 4-H Foundation, Inc. Reimbursements of 96% will be made for entry cancellations received before Friday, September. 12, 2008. After this date, only 50% of your entry fee will be refunded until September 26, 2008.
