Registration

• Costs: Executive Board Registration Fee is $90.00 per participant. No registration fee refunds. Checks must be made payable to the Florida 4-H Foundation. Only one county check from each county should be sent.
• Forms Needed: Your registration must consist of a Registration Form, Florida 4-H Participation Form, and one Agent Responsibility Form per county. You do not need to resubmit your Florida 4-H Participation Form if it was submitted for 1st or 2nd Executive Board. Participation Forms must be on file for all participants, youth and adults.

• Registration Deadline: This form must be submitted to your county 4-H extension office by their set deadline. County offices should forward all registration materials and one county check to the State 4-H Office by the posted deadline.
